Therapeutic unfractionated heparin

A few recent papers by our group demonstrate that a (slightly modified) version of this protocol decreased the incidence of post-surgical bleeding, and that an aPTT >150 is associated with an increased risk of bleeding.

Goal Anti Xa Fondaparinux level 0.5 to 1.5. Measure 3 to 4 hours after 2nd dose.
