Therapeutic unfractionated heparin
A few recent papers by our group demonstrate that a (slightly modified) version of this protocol decreased the incidence of post-surgical bleeding, and that an aPTT >150 is associated with an increased risk of bleeding.

Treatment for DVT, PE, HIT
|< 18 years old and < 50 kg||0.1 mg/kg once daily|
|< 18 years old and ≥ 50 kg||5 mg|
|≥ 18 years old and < 50 kg||5 mg|
|≥ 18 years old and 50-101 kg||7.5 mg once daily|
|≥ 18 years old and ≥ 101 kg||10 mg|
Goal Anti Xa Fondaparinux level 0.5 to 1.5. Measure 3 to 4 hours after 2nd dose.
